Scholarships & Criteria
The following is a current list of the South Madison Community Foundation Scholarships, and the criteria considered in each scholarship selection process.
Arabian Wrestling Club Scholarship
• PHHS senior wrestler who has been active in wrestling for a minimum of two years
• Pursuing a two- or four-year degree at an accredited institution
• Cumulative GPA of at least a 7.0 on a 12.0 grade scale
• Demonstrate good character as well as positive leadership potential in wrestling, other activities, and in the community
• One reference letter (form provided)
Blake M. Bennett Memorial Scholarship
• PHHS graduate pursuing a vocational career in the area of cosmetology from any accredited cosmetology school in the State of Indiana
• Essay (500 words or less) as to why you selected cosmetology as your chosen career (form provided)
• Complete a College Costs Estimator form and corresponding release form (even if one has been completed previously) by going to www.indianacollegecosts.org/project/southmadison2013
George S. and H. Loma Berkebile Scholarship
*Applications available at the Anderson campus of Ivy Tech State College
• Graduate of any high school in the Anderson Community School System
• Pursuing vocational training at an accredited institution
• Evidence of work ethic
Larry L. Blubaum Scholarship Fund
*Special application is required.
• Mount Vernon (Indiana) Senior High School senior pursuing postsecondary training at an accredited public or private institution
• Essay entitled “My educational and career goals”
• Two reference letters
• Applicants should consult Guidance Department counselor(s) for further instructions and application form.
M. Brown Memorial Scholarship
• PHHS senior pursuing a two- or four-year degree in the construction industry at an accredited institution
• Cumulative GPA of “B” or better

Kevin Carey Memorial Scholarship
• PHHS senior who has been active in the PHHS cross country and/or track running program(s)
• Demonstrate leadership and sportsmanship
• Academics
• Participation in community and school activities
• Essay (700 words or less) – “How Running Has Impacted My Life Outside The Sport” (form provided)
• Coach's reference (form provided)
Brenda J. Cooksey Memorial Scholarship
• PHHS senior pursuing postsecondary vocational training or a two-year associate degree at an accredited institution
• Regular or Core 40 diploma
• Cumulative GPA of 6 or better on a 12-point grading system
• Work ethic as evidenced by summer and/or year round employment
• Two reference letters: one from current/past employer and one from a teacher/school official (form provided)

Chris Coyle Memorial Scholarship
• Active in PHHS Choral Department
• Pursuing a postsecondary degree at an accredited institution
• Demonstrate moral character and evidence of church, school and community commitment (form provided)
• Participation in extracurricular activities
• Showing academic potential and achievement as indicated by high school records
• Two addendums (form provided)
Bill and June Creed Memorial Scholarship
• LHS senior pursuing a four-year degree in business (including, but not limited to accounting, economics, finance) or language arts (English, foreign language, literature) at an accredited institution
• Cumulative GPA of “B” or better
• Evidence of a strong work ethic and active community service
Elizabeth Edmundson Memorial Music Scholarship
• PHHS student pursuing a four-year college degree
• Criteria in order of priority:
o PHHS student who plays piano and will pursue a degree in a music discipline.
o PHHS student who has been a member of band or choir and will pursue a degree in a music discipline.
o PHHS student who has been a member of band or choir and will pursue a degree in education.
• Evidence of volunteer or community service
• Essay (500 words or less) – “How Music Has Impacted My Life” (form provided)

Jennifer C. Fox Memorial Scholarship
• PHHS senior pursuing postsecondary degree at an accredited Indiana public college/university
• Cumulative GPA of between 5.0 and 9.0 on a 12-point scale
• Must emulate Jennifer's freedom of thought and love of life
• Ability to clearly express thoughts and ideas through effective writing (form provided)
• Academic performance and achievement in liberal arts
• Two reference letters: one personal and one academic (form provided)
Future of Agriculture Scholarship
*Check with Foundation office—this scholarship is not awarded every year.
• Senior of Lapel High School or PHHS pursuing a postsecondary degree in an Agricultural-related field at an accredited institution
• Cumulative GPA of “C” or better
• Evidence of extracurricular agricultural activities (4H, FFA, etc.)

Charles L. Godsey Memorial Athletic Scholarship
*Students receiving athletic scholarships are NOT eligible to apply.
• PHHS senior active in the PHHS sports program who has received at least two varsity letters in one or more athletic programs
• Cumulative GPA of 7 or better on a 12-point scale
• Evidence of good mental attitude and work ethic
• Two reference letters: one character and one coach's (form provided)

Donald Gwinn Memorial Scholarship
• LHS senior pursuing a four-year degree at an accredited institution
• Cumulative GPA of “B” or better
• Career plans in an agricultural-related or scientific field
• Evidence of a strong work ethic and active community service
• Essay (500 words or less) outlining future goals in agriculture or science (form provided)
Tyler J. Hatheway Memorial Scholarship
• PHHS senior pursuing a four-year degree at an accredited institution
• Cumulative GPA of “B” or better
• Demonstrate consistent, long-term and regular involvement and dedication to their local church; show evidence of strong and favorable high character; and show evidence of care and concern for others (a “team player”) – qualities that are to be addressed by the completed addendum
• Essay (250 – 500 words) – EITHER "Excluding your immediate family, who in your life has had the most influence on your character and why?" OR “When your life draws to a close, what criteria will you use to measure the successfulness of your life and why?" (form provided)
• Two reference letters (form provided)
Jessica D. Henson Memorial Scholarship
• Female active in the Pendleton Jr. Softball Association Program
• Pursuing a postsecondary degree at an accredited institution
• Evidence of church, school and community commitment (form provided)

• PHHS senior pursuing a four-year degree related to the physical/life sciences at an accredited institution

H. Ross Jackson and Elizabeth S. Jackson
• PHHS senior pursuing a four-year degree in education at an accredited Indiana institution
• Cumulative G.P.A. of “B” or better
• Participation in extracurricular activities and community service
• One reference letter from a teacher/school official addressing the applicant's potential to complete undergraduate studies (form provided)

Patricia H. Kline Memorial Scholarship
• Must be in the top 25% of the PHHS graduating class
• Entering the field of social work, religious studies or performing arts at an accredited institution
• Active in dance/theatre/chorus and church-related activities
• One-page essay entitled “Two Lessons I've Learned from My Parents That I Will Pass To My Children and Others" (form provided)
• Reference letter from church pastor highlighting church activities (form provided)
Kelson L. Lineberry Memorial WEEM Scholarship
• PHHS senior active in WEEM radio program
• Pursuing a postsecondary degree at an accredited institution
• Cumulative GPA of “B” or better
• Participation in extracurricular activities
David L. Miller Scholarship Fund
*A special application is required.
• Mitchell (Indiana) Senior High School senior
• Financial eligibility based on median household income of Lawrence County (please see school counselor for details)
• Biography (300-500 words) outlining significant accomplishments, persons who have been an influence, and a statement of educational/career goals

Jill D. Mollenkopf Memorial Scholarship
• PHHS female senior who has played four continuous years of high school softball and earned a varsity letter at least two of those years
• Pursuing a postsecondary degree at an accredited Indiana institution
• Evidence of school and community commitment
• Participation in extracurricular activities
• Demonstrates exemplary leadership characteristics and school spirit
• Two reference letters: one personal and one academic (form provided)

Sandra A. and Mary E. ‘Beth' Neese Memorial Scholarship
• LHS senior
• Cumulative GPA of 2.5 or higher on a 4-point scale
• Priority will be given to students who demonstrate an interest in public service, law or volunteering for organizations such as the Peace Corps or Americorps
• Evidence of leadership through community service and/or extracurricular activities
• Two reference letters: one addressing the applicant's character and one addressing academic success and potential (form provided)
• Essay (500 words or less) explaining why you think you deserve the scholarship (form provided)
PHHS Memorial Scholarship
• PHHS senior pursuing a postsecondary degree at an accredited institution
• Potential to complete undergraduate studies (form provided)
• Participation in extracurricular activities and community service
• Exhibit moral character
• Two reference letters: one addressing the applicant's character and one addressing academic success and potential (form provided)
Pendleton Optimist Club Scholarship
• PHHS senior pursuing additional education through vocational/technical training
• Cumulative GPA of “C” or better for a graduating
• Participation in extracurricular activities and community service
• One reference letter from a teacher/school official addressing the applicant's potential to complete undergraduate studies (form provided)
Randolph/Fraternal Order of Police Scholarship
• Must have a parent/grandparent who is a member of F.O.P. #48. Include copy of membership card.
• Senior from Anderson, Lapel, Pendleton, Alexandria, Elwood or Frankton High Schools
• Pursuing an undergraduate degree from an accredited institution

Jeremy Rodecap Memorial Scholarship
• Senior of the South Madison Community School Corporation and/or a resident of Fall Creek, Adams or Green Township, Madison County, IN
• Pursuing postsecondary training in the field of Emergency Medical Services, Emergency Medical Technician or Paramedic OR second choice of education in the Medical Service field
• Training must take place at an accredited institution
• Cumulative GPA of 2.5 or higher on a 4 point scale (seniors only)
• Evidence of community service and active church affiliation

Howard L. and Elva Fern Simmerman Memorial Scholarship
• PHHS senior or graduate—priority given to PHHS seniors
• Pursuing a four-year education degree at an accredited institution
• Cumulative G.P.A. of 2.5 or higher on a 4-point scale
• Evidence of leadership through community service and/or extracurricular activities
• Evidence of work ethic
